Nurse Clinical Manager – Broward County

Activa Home Health (an Accord Care Company) is a licensed Medicare‑certified home healthcare agency seeking an enthusiastic, customer‑service–driven RN Clinical Manager for our Broward office. The position primarily works in the office and also performs field case management, including assessment, coordination of care with other disciplines, and communication with referral sources. The role requires strong case‑management experience and working knowledge of Medicare.

• Perform case‑management oversight and coordination of care for all skilled disciplines.
• Maintain close communication with the social worker and veterans division.
• Coordinate patient admissions with other members of the healthcare team.
• Provide nursing care and treatment of home‑care patients in accordance with the plan of treatment and approved authorizations.
• Supervise all non‑skilled caregivers.

• Current RN Florida professional license.
• Minimum of one year of experience with Medicare home‑health case management.
• Excellent time‑management skills to prioritize and organize workload.
• Positive attitude and strong work ethic.
• Excellent communication and customer‑service skills.
• Experience with Kinser Wellsky is a plus.
