Community Outreach

Thrive & Success Community Outreach is committed to strengthening children and families through trauma-informed, family-centered services. The HEAL Trauma Program provides intensive case management, care coordination, advocacy, and resource navigation to children and caregivers experiencing trauma and significant life stressors. The program is funded through the Children’s Services Council (CSC) of Broward County under the HEAL Trauma Program contract.

Position Summary

The Community Mental Health Worker (CMHW) is responsible for providing trauma-informed, community-based case management and care coordination to children and families enrolled in the HEAL Trauma Program. The CMHW serves as an advocate, educator, and resource navigator while helping families identify strengths, overcome barriers, increase stability, and achieve individualized goals.

This position requires extensive field work, including home visits, school visits, community outreach, and collaboration with multidisciplinary partners.

Essential Job Responsibilities

  • Maintain an active caseload of families participating in the HEAL Trauma Program.
  • Conduct community-based and home visits with enrolled families.
  • Complete program intake, eligibility verification, and enrollment.
  • Conduct comprehensive family assessments and identify family strengths and needs.
  • Develop individualized Family Success Plans with measurable goals.
  • Assist families in accessing:
  • Mental health services
  • Medical care
  • Housing resources
  • Food assistance
  • Employment resources
  • Childcare
  • Educational supports
  • Transportation
  • Financial assistance
  • Other community resources
  • Provide advocacy and care coordination with schools, healthcare providers, behavioral health providers, child welfare agencies, and community partners.
  • Maintain regular contact with families to monitor progress toward established goals.
  • Assist families during crises by providing immediate support and appropriate referrals.
  • Participate in multidisciplinary staffing and case consultations.
  • Conduct follow-up services to ensure successful linkage to community resources.
  • Participate in community outreach events, health fairs, and networking activities.
  • Maintain professional relationships with community partners to expand available resources.

Documentation Responsibilities

Maintain accurate, timely, and complete documentation, including:

  • Home visit documentation
  • Community outreach documentation
  • Client satisfaction surveys
  • Required program forms
  • Performance measure documentation
  • SAMIS data entry
  • Monthly reporting requirements

Documentation must be completed within agency and funding timelines and maintained in accordance with CSC requirements. The CSC contract requires documentation, client records, personnel records, time records, SAMIS reporting, and other required program documentation to be maintained and available for review and audit.

Required Qualifications

Education

Bachelor’s degree in one of the following:

  • Social Work
  • Psychology
  • Human Services
  • Sociology
  • Counseling
  • Public Health
  • Family Studies
  • Criminal Justice
  • Related behavioral health field

Experience

Preferred:

  • One to two years working with children and families
  • Community-based case management
  • Trauma-informed services
  • Behavioral health
