• Assisting patients with activities of daily living
• Obtaining and recording vital signs, weights and heights
• Assisting patients with mobility: ambulation, transfer, discharges and ROM activities
• Performing post mortem care
• Measuring and recording input and output (I & O)
• Performing EKGs, phlebotomy and blood glucose monitoring
• Observing and reporting changes in patients’ condition to the RN
• Maintaining phlebotomy equipment and inventory; performing other related duties as assigned

• High School Diploma or GED
• BLS required within 3 months of hire

• Interpersonal skills to effectively communicate and collaborate with other members of the Care Center
• 6 months prior acute care experience strongly preferred
• CNA or CPCT strongly preferred
