Job Description
Responsible for working alongside multiple internal/external physician and health care professionals and facilitating advanced primary and specialty nursing services. Provides general and preventative care for adults at the end-of-life, treat illnesses, discuss goals of care, order lab tests, and prescribe medication in the inpatient and outpatient/ambulatory setting.
Principal Accountabilities- Provides new patient consultation and reviews health history.
- Performs physical examinations and screening on patients.
- Provides after-care/ongoing care information and resources to patients and their families.
- Ensures patient records are maintained including test results, immunizations, condition changes.
- Delivers quality, advanced practice care while complying with company policies, safety regulations, and productivity metrics.
- Recommends further treatment options or future appointments if required.
- Performs diagnosis of health problems and provides medications.
- Performs other related duties as assigned.
- Master’s degree in Nursing in area of specialty practice; or post‑master’s certificate in area of specialty practice with graduate degree in nursing
- Current state APN certificate
- Current RN license
- 5 or more years experience in Nursing or relevant area.
- Medical, Dental, Vision, Prescription Coverage (22.5 hours per week or above for full-time and part-time team members)
