Overview
The wound care nurse applies the Nursing Process to the provision of care for patients with acute and chronic wounds, ostomies, and, where applicable, patients undergoing hyperbaric oxygen therapy (HBOT). The wound care nurse coordinates treatment plans and collaborates with interdisciplinary teams to achieve optimal outcomes in both inpatient and outpatient settings. The nurse maintains comprehensive clinical documentation, adheres to safety protocols, and provides patient education and advocacy.
PrincipalAccountabilities
Job Responsibilities / Essential Functions include (but are not limited to):
- Provides direct care to inpatients and outpatient ostomy patients with application of the Nursing Process. Performs patient assessments, implements care plans, and adheres to established standards of care.
- Collaborates with the Manager in areas of daily operational and fiscal management. Assists in the development of wound and ostomy care policies and procedures. Able to assume super-user status when implementing new processes/equipment.
- Actively interfaces and maintains a collaborative relationship. Collaborates with physicians, APPs, fellow wound care nurses, wound program leadership, and other hospital staff to ensure service excellence.
- Coordinates with Manager to maintain a safe environment of care. Adheres to OSHA, CMS, CDC, Joint Commission, NJ Department of Health, and applicable medical center standards and policies.
- Case Management. Identifies, coordinates, and meets patient care needs including home care, DME orders, and communication with all providers involved in the patient s care.
- Serves as a clinical resource to the health care team, patients, and families. Acts as a mentor/preceptor. Provides education and consultation to staff nurses, physicians, APPs, and support staff for wound, skin and ostomy care concepts, practices, products and devices. Participates in hospital and system-wide research and educational programs and studies.
Required
- Graduate of an accredited RN Program (All ASN must complete their BSN studies within five years of their start date).
- Current state licensure as a Registered Nurse
- BLS certification
- ACLS certification (Hyperbaric only)
Preferred
- 1+ year experience in acute care nursing and/or wound/HBO care preferred
- Wound and/or Hyperbaric Certification strongly preferred (WOCN, WCC, CWS, CAWS, ACH, CHT, CHRN)
- Bachelor’s degree in nursing
