Job Description

Atlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center.

• Provide Social Work coverage to facilitate discharges, meet with patients and families to assess and provide discharge planning support, resources, referrals, and transfers to Extended Care and inpatient psychiatric facilities.
• Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, and communicate with community resources and area facilities.
• Phone, fax and electronic documentation are required.

Minimum Education:

MSW and LSW

Minimum Experience:

1‑year medical setting (acute care hospital) or 1‑year externship would be preferred
