PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Ponca City, Oklahoma.

Position Overview

Seeking an experienced Registered Nurse to provide skilled nursing care to patients in their homes. This role focuses on delivering high-quality, patient-centered care across the Ponca City area and surrounding communities. The ideal candidate has strong home health experience, is comfortable working independently in the field, and demonstrates excellent clinical judgment.

Schedule & Work Details
  • Location:

    Ponca City, OK and 50-mile radius coverage area
  • Schedule:

    Monday through Friday, 08:00 to 16:30
  • Weekly Guarantee: 40 hours
  • On-Call/Weekend Requirement: 1 weekend per month
  • Mileage Reimbursement:
    Yes, between patient visits (first to last patient)
  • Pay Rate: $51 to $53 per hour
Key Responsibilities
  • Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
  • Complete patient assessments, develop care plans, and update documentation using HCHB (Homecare Homebase)
  • Administer medications and treatments, including wound care as needed
  • Monitor patient conditions and communicate changes to the care team and physician
  • Educate patients and families on disease management, medications, and home care techniques
  • Maintain accurate, timely, and compliant documentation
  • Coordinate care with interdisciplinary team members to ensure optimal patient outcomes
Required Qualifications
  • Active Registered Nurse (RN) license in the state of Oklahoma
  • Current CPR certification
  • Prior home health experience required
  • Experience with Homecare Homebase (HCHB) required
  • Strong assessment, documentation, and communication skills
  • Ability to work independently and manage a field-based schedule
