Licensed Practical Nurse - Per Diem

Overview

Per Diem Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.

• Provide services within the scope of practice as defined by state nursing laws, in accordance with the plan of care, and in coordination with other members of the health care team. May not alter the plan of care.
• Instruct the patient’s family in caregiving and maintenance of a healthy environment for the patient.
• Observe, record, and report to the supervising nurse and/or physician the patient’s response to treatment and changes in the patient’s condition daily.
• Function effectively in any home setting, regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Note:

This role may be eligible for various compensation components (per visit point pay and hourly pay) depending on work performed and productivity targets.

Required Qualifications

• Current and unrestricted LPN or LVN licensure in the state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire.
• Home care experience.
• Flexible hours; ability to work independently.
• Good communication, writing, and organizational skills.
