Job Title: RN – Home Health
Location: Hammond, LA 70403
Duration: 3 months+ (possibilities of extension)
Shift: Days, Monday-Friday (8:00 AM – 4:30 PM)
Job Responsibilities- Admission oasis visits, recertification visits, resumption of care visits, discharge visits, transfer documentation
- Regular skilled nursing visits, wound care, wound vac, drain care including pleurx
- Catheter care (indwelling and suprapubic), IV maintenance, venipuncture
- Central line dressing care, lab draws with delivery to appropriate lab for processing
- Ostomy care, staple/suture removal
- Proper communication and coordination of care with other staff and physician offices
- Timely completion and return of documentation, assessment, education and medication reconciliation
- Bachelor's Degree
- Home health RN experience including documentation experience in HCHB
- RN license in Louisiana and BLS or ACLS certification
- Productivity expectations: 30 points per week
- On Call/Weekend Requirement:
No - Coverage Areas: primarily Tangipahoa, possibility of needs in St Tammany
- Dress code: scrubs, any color but preferably royal blue
