The Case Management Coordinator - Low Intensity role provides case management services to individuals whose behavioral health needs are stable and primarily supported through medication management, routine monitoring, and coordinated care. This role focuses on maintaining engagement, completing required assessments and documentation, coordinating services, and supporting recovery and wellness in collaboration with the interdisciplinary care team. Services are designed for individuals who benefit from ongoing oversight and support without the need for intensive clinical intervention or crisis-driven services.
- Demonstrates the ability to engage individuals in a welcoming, hopeful, and empathic manner regardless of disability or phase of recovery.
- Maintains regular contact with assigned individuals, meeting with each person at least quarterly to monitor stability, medication adherence, and continued engagement in services.
- Provides brief, recovery-oriented support, education, and coaching related to medication management, wellness strategies, and self-management skills.
- Coordinates care with psychiatrists, nurses, therapists, and other internal providers to support continuity of services, including facilitating appointments, medication refills, and communication across the care team.
- Completes required assessments and documentation accurately and within established time frames, including annual LOCUS and SDOH assessments, in accordance with CCBHC, Medicaid, and agency requirements.
- Monitors changes in functioning, engagement, or risk indicators and follows established workflows for consultation, reassessment, or referral to higher-intensity services when clinically indicated.
- Assists individuals in identifying and accessing appropriate supports and resources, including transportation, benefits, peer supports, and other community-based services to promote stability and independence.
- Addresses non-clinical barriers to engagement and supports individuals in building self-efficacy, independent living skills, and recovery-oriented goals.
- Maintains documentation in the electronic health record in compliance with agency standards, regulatory requirements, and payer guidelines.
- Participates in team meetings, case consultations, and supervision to review caseload status, share relevant information, and support coordinated care delivery.
- Ensures paperwork is maintained in accordance with Medicaid, Department of Community Health, and Easterseals MORC policies and procedures.
- Maintains up-to-date knowledge of Medicaid Provider guidelines and other applicable regulatory standards.
- Travels to various locations throughout Oakland County to assess individual needs and support service coordination.
- Demonstrates knowledge of principles of trauma-informed care.
- Provides support and assistance to individuals experiencing trauma-related issues.
