Overview

Graham Healthcare Group is seeking an RN Home Health to join our residential team in Pontiac, Michigan. The position is ongoing, 40 hours per week, 8‑hour shifts, and is staff‑type. You will support patients in Auburn Hills, Pontiac, Bloomfield/Bloomfield Hills, West Bloomfield, Orchard Lake, Sylvan Lake, Birmingham, Southfield, and surrounding areas.

Responsibilities

As a home health registered nurse case manager, you will:

  • Lead and collaborate to manage and deliver patient care plans.
  • Provide clinical oversight and support for patients and family caregivers.
  • Maintain accurate charting and meet critical deadlines.
  • Manage your own time and schedule, planning visits and responding to call requests.
Qualifications
  • RN license in the state you work.
  • Graduated from an approved school of practical nursing.
  • One year of RN experience and clinical competence to deliver quality patient care.
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
  • Commitment to consistently meet critical charting deadlines.
  • Experience self‑managing time and schedule.
  • Proficiency with tablets, mobile phones, and EMR software.
