About the Department

As part of Henry Ford Health's Community Care Services, our Home Dialysis team plays a vital role in expanding access to care by delivering essential health services directly to patients in their homes. This department focuses on meeting patients where they are, providing compassionate and coordinated care across diverse populations. Through home health care and specialized dialysis services, we ensure patients receive the support and resources they need to manage their health effectively.

Role Overview

As an RN in Home Dialysis, you will lead the assessment, planning, and implementation of personalized training strategies for patients and their families. Your role is critical in ensuring the delivery of quality, compassionate care by continuously evaluating patient needs and coordinating care. You will utilize the nursing process to provide, delegate, supervise, and document care, making a direct impact on patient outcomes and quality of life.

What You Will Do

• Assess patient and family needs, including environmental assessments, to develop tailored training strategies for home dialysis.
• Plan and implement ongoing patient care, ensuring continuous re-evaluation at regular intervals.
• Provide leadership in coordinating and delivering compassionate, quality patient care.
• Utilize the nursing process to assess, plan, implement, and evaluate care, including teaching patients and families.
• Travel periodically to patient homes to provide necessary care and support.

What You Will Need

Required Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in the State of Michigan.
• At least 12 months of experience as a registered nurse, plus 3 months of experience in hemodialysis (HD) and/or peritoneal dialysis (PD).
• If responsible for both HD and PD programs, 12 months of nursing experience plus at least 3 months in each modality.
• Basic computer knowledge and keyboarding skills.

Preferred Qualifications

• Experience in the specialty.
