Responsibilities- Evaluate and treat residents in an assisted living community and or Home setting
- Develop and implement individualized treatment plans
- Collaborate with physicians, families, and the interdisciplinary care team
- Maintain accurate, timely documentation in the EMR
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
- Active Florida Physical Therapist (PT) license
- New graduates and experienced therapists are encouraged to apply
