LPN - 7pm-7am Shift

Resorts at Pooler is seeking a dedicated and compassionate LPN to join our team at our Nursing Home in Pooler, Georgia. As a LPN on our 7pm-7am shift, you will play a vital role in providing high-quality care to our residents while ensuring a smooth transition between shifts. • Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
• Conduct resident assessments and develop individualized care plans in collaboration with the interdisciplinary care team
• Support residents with daily living activities, such as bathing, dressing, and feeding
• Monitor and report any changes in resident condition to the nursing staff and/or physicians
• Maintain accurate and up-to-date resident records and documentation
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services
• Perform other duties as assigned by the Nurse Manager or designee

• Currently licensed as a Licensed Practical Nurse (LPN) in the State of Georgia
• High school diploma or equivalent required
• Minimum of 0 years of experience as an LPN (new graduates welcome to apply)
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Ability to lift, transfer, and reposition patients as needed
