Your Health is seeking a Registered Nurse to service our Wound Care patients throughout the Savannah area. The Visiting Nurse (Nurse Case Manager) travels to patients’ homes and facilities to evaluate care needs and coordinate with providers, including telemedicine visits, as part of an integrated care team.
This full-time role requires reliable transportation and 10-hour shifts Monday-Thursday (7am-6pm). RN license preferred; three years of clinical and community health experience are valued.
