Licensed Practical Nurse – In-Home Services (Healthcare)

PRN position

We are seeking a compassionate, dependable, and detail-oriented Licensed Practical Nurse (LPN) to join our In-Home Services team. In this role, you will provide nursing support under the direction of a Registered Nurse (RN), helping ensure participants receive safe, high-quality care in their homes. The LPN plays an important role in reinforcing caregiver training, monitoring participant health, conducting supervisory visits, and communicating clinical observations to the RN.

• Conduct interim supervisory visits and participant condition checks under the direction of the RN.
• Reinforce aide and Certified Nursing Assistant (CNA) skills following the RN's initial delegation and observe return demonstrations.
• Collect and report clinical information, including vital signs, skin integrity, bowel and bladder patterns, and other participant observations.
• Complete supervisory documentation accurately and submit findings to the RN for review.
• Participate in incident investigations and assist with corrective action follow-up.
• Educate CNAs and aides on infection prevention, safety practices, documentation, and reporting procedures.
• Support the RN with admissions, recertifications, and other clinical documentation as assigned.
• Ensure services are provided in accordance with agency policies, participant Plans of Care, and applicable Missouri regulations.

• Current Licensed Practical Nurse (LPN) license in the State of Missouri in good standing.
• Minimum of one (1) year of experience in home health, personal care, or long-term care preferred.
• Strong clinical assessment, documentation, delegation, and communication skills.
• Current CPR and First Aid certification.
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Ability to pass required background screening.
