Home Health Case Manager LPN

The Home Health Licensed Practical Nurse (LPN) Case Manager provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse and in accordance with physician orders, the individualized Plan of Care, and Medicare Conditions of Participation. The LPN collaborates with the interdisciplinary team to coordinate patient care, reinforce patient education, monitor patient progress, identify changes in condition, and communicate findings to the RN Case Manager and physician to promote quality patient outcomes.

• Provide skilled nursing visits as ordered by the physician and directed by the RN Case Manager.
• Administer medications, injections, IV therapy (if permitted by state law), and treatments as prescribed.
• Perform wound care, dressing changes, catheter care, ostomy care, tracheostomy care, enteral feeding support, and other skilled nursing procedures within the LPN scope of practice.
• Monitor and document patient vital signs, pain levels, respiratory status, cardiovascular status, skin integrity, nutritional status, and overall health.
• Observe for changes in patient condition and immediately report significant findings to the RN Case Manager and physician.
• Reinforce patient and caregiver education regarding disease processes, medications, safety precautions, infection prevention, diet, and self-care.
• Evaluate the effectiveness of treatments and interventions and communicate findings to the supervising RN.

• Collaborate with the RN Case Manager in implementing and updating the patient's Plan of Care.
• Coordinate services with physicians, therapists, medical social workers, home health aides, and other members of the interdisciplinary team.
• Participate in interdisciplinary case conferences and care planning meetings.
• Assist in obtaining physician orders and communicating care updates.
• Ensure continuity of care during transitions between healthcare settings.
• Follow all Medicare Conditions of Participation for Home Health Agencies.
• Assist the RN Case Manager with gathering clinical information necessary for OASIS documentation.
• Recognize that only qualified clinicians may complete comprehensive assessments and OASIS documentation as permitted by Medicare regulations.
• Ensure skilled nursing documentation supports medical necessity and homebound status observations.
• Complete visit documentation accurately and within agency-required time frames.
• Maintain compliance with HIPAA, OSHA, infection control standards, and agency policies.
• Reinforce physician and RN teaching plans.
• Educate patients and caregivers regarding:
  • Medication administration and compliance
  • Disease management
  • Wound care
  • Diabetes management
  • Cardiac and pulmonary care
  • Fall prevention
  • Infection prevention
  • Equipment use
  • Emergency preparedness
• Verify patient and caregiver understanding through return demonstration and teach-back methods.

• Complete all clinical documentation in the electronic medical record (EMR) on the day of service whenever possible.
• Document patient responses to treatment, interventions provided, education delivered, and communications with physicians and the interdisciplinary team.
• Report significant changes in patient condition immediately according to agency policy.

• Participate in agency Quality Assessment and Performance Improvement (QAPI) activities.
• Attend mandatory staff meetings and continuing education programs.
• Maintain competency through annual skills validation and required in-service education.

• Graduate of an accredited Practical Nursing program.

• Current unrestricted Licensed Practical Nurse (LPN) license in the state of Missouri.

• Minimum of one year of nursing experience preferred.
• Previous home health, hospice, medical-surgical, rehabilitation, or long-term care experience preferred.
• Experience with Medicare home health documentation and electronic medical records preferred.

• Current Basic Life Support (BLS/CPR) certification.
• Valid driver's license.
• Reliable transportation with current automobile insurance.

• Knowledge of Medicare Home Health Conditions of Participation.
• Understanding of home health regulations and standards.
• Knowledge of infection prevention and control practices.
• Strong clinical observation and assessment skills within the LPN scope of practice.
• Excellent communication and patient education skills.
• Ability to work independently while maintaining frequent communication with the RN Case Manager.
• Strong organizational and time management skills.
• Proficiency with electronic medical record systems.

• Frequently stand, walk, bend, kneel, crouch, and reach.
• Lift, push, or pull up to 50 pounds with or without reasonable accommodation.
• Assist patients with transfers and mobility as appropriate.
