Registered Nurse – Home Health

Job Summary:

Provides direct nursing care to home health patients in accordance with physicians’ orders and agency policies. Coordinates patient care activities, supervises home health aides and licensed practical nurses, and ensures compliance with clinical protocols.

Responsibilities:

• Administer comprehensive nursing assessments and develop individualized care plans.
• Coordinate and document home visits, wound care, and follow-up services.
• Supervise and evaluate home health aides and licensed practical nurses to ensure quality care.
• Maintain accurate and timely documentation using Home Care Home Base EMR system.
• Ensure compliance with agency policies, protocols, and regulatory standards.
• Manage travel responsibilities within a designated geographic area, covering up to 75 miles from the clinic.

Qualifications:

• Hold a current valid RN license in the state of practice.
• Demonstrate at least 1 year of recent adult home health or hospice experience.
• Proficient in OASIS documentation for RNs or physical therapists and occupational therapists.
• Possess a valid driver’s license, reliable transportation, and agency-required liability insurance.
• Willing to travel up to 75 miles from the clinic site.
• Capable of working independently with excellent clinical and communication skills.
