If you're detail-oriented, have excellent communication skills, and are passionate about advocating for others, this role offers an opportunity to directly impact our community's this vital role, you'll work closely with clients to assess their housing needs, coordinate housing placements, and provide ongoing support to help them maintain stable housing.
You'll collaborate with our clinical teams, community partners, and the DBH Housing Unit to ensure comprehensive support for our clients' recovery journeys.

* Key Responsibilities:
*** Coordinate and facilitate housing searches, placements, and success coaching for clients
* Assess clients' housing needs, strengths, and resources to develop effective support strategies
* Advocate for expanded housing resources and collaborate with community partners
* Provide experiential training in self-advocacy skills and resource acquisition
* Participate in clinical team meetings to contribute to client recovery planning
* Monitor external services to ensure coordinated care
* Maintain knowledge of community recovery resources and support systems
* Ensure protection of client rights and dignity
* Complete necessary documentation and reporting requirements
** Education, Experience, and/or Credential
Qualifications:
*** Bachelor's Degree from an accredited college or university in social work, psychology, nursing or a closely related field
* Equivalent experience may be substituted on the basis of one year for each year of required educational training with a base education of a High School Diploma or GED
*
* Additional Qualifications:
*** Must complete background check including criminal record, driving record, abuse/neglect and fingerprint check
* Current driver's license, acceptable driving record and current auto insurance required
* Strong communication and interpersonal skills
* Ability to build effective relationships with clients and community partners
* Problem-solving skills and ability to work independently
* Must complete all required training at the beginning of employment and annually thereafter
