Position Summary

We are seeking an experienced and compassionate Hospice RN Case Manager to provide high-quality hospice care services throughout Baker County. This position is responsible for managing a patient caseload, completing hospice admissions, coordinating interdisciplinary care, and supporting patients and families through the end-of-life journey.

• Manage and coordinate hospice patient care plans.
• Conduct comprehensive patient assessments and ongoing evaluations.
• Complete hospice admissions and determine eligibility requirements.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Provide education and emotional support to patients and families.
• Maintain accurate and timely clinical documentation using Point Care.
• Ensure compliance with hospice regulations and organizational standards.

• Active RN license.
• Previous hospice nursing experience.
• Experience using Point Care.
• Strong clinical assessment and case management skills.
• Valid driver's license and reliable transportation.

• Compassion for rural patients and families requiring additional support.
• Excellent communication and organizational skills.
• Ability to work independently while collaborating effectively with the care team.

• One weeknight on-call every other week.
• Weekend call rotation shared among four nurses.

• Professional scrubs.
• No specific scrub color requirement.
