Position Summary
We are seeking an experienced and compassionate Hospice RN Case Manager to provide high-quality hospice care services throughout Baker County. This position is responsible for managing a patient caseload, completing hospice admissions, coordinating interdisciplinary care, and supporting patients and families through the end-of-life journey.
Key Responsibilities- Manage and coordinate hospice patient care plans.
- Conduct comprehensive patient assessments and ongoing evaluations.
- Complete hospice admissions and determine eligibility requirements.
- Collaborate with physicians, caregivers, and the interdisciplinary care team.
- Provide education and emotional support to patients and families.
- Maintain accurate and timely clinical documentation using Point Care.
- Ensure compliance with hospice regulations and organizational standards.
- Active RN license.
- Previous hospice nursing experience.
- Experience using Point Care.
- Strong clinical assessment and case management skills.
- Valid driver's license and reliable transportation.
- Team-oriented mindset.
- Compassion for rural patients and families requiring additional support.
- Excellent communication and organizational skills.
- Ability to work independently while collaborating effectively with the care team.
- One weeknight on-call every other week.
- Weekend call rotation shared among four nurses.
- Professional scrubs.
- No specific scrub color requirement.
