Assured Home Health & Hospice, part of LHC Group, is seeking a Hospice Nurse Practitioner to provide palliative care, symptom management and supportive care for Hospice patients as requested by the referring physician.
Responsibilities include face-to-face visits, coordinating the plan of care, documenting patient progress, and collaborating with the IDG. Requires unrestricted license in the state of residence, NP or CNS certification, CPR, and 5+ years of nursing experience.
