• Home health setting with patient coverage in Port Charlotte, Englewood, Venice, Nokomis, Osprey, and Sarasota
• Geriatric patient population
• Average caseload of 5 to 7 patients per day
• Day shift schedule, Monday through Friday, typically 8:00 AM to 5:00 PM
• No on-call requirements
• Territory can extend up to a 60-mile radius from the home base
• EMR system is HCHB; prior experience with this system is required
• Oasis documentation experience required
• Mileage is reimbursed at the IRS standard rate
• iPad provided for work

• Current state PT license required if applicable
• Minimum 1 year of recent home health physical therapy experience
• Current BLS certification required
• Experience with HCHB EMR required
• Experience with Oasis documentation required
• Flexibility to travel within the assigned territory
• Must be able to wear scrubs in any color (no jeans)

• Main responsibilities include providing physical therapy services to geriatric patients in their homes
• First-time travelers are welcome to apply
• Local candidates are accepted at the travel rate
• Scrubs are required and must not include jeans
• Mileage reimbursement and iPad provided to support mobile work
