Job Description Summary
Provides comprehensive primary, acute, and extended care services in accordance with scope of practice, licensure, and the Collaborative Practice Agreement. Conducts assessments, diagnoses health conditions, implements clinical interventions, and evaluates patient responses to care. The role supports clinical operations by coordinating patient care in collaboration with physicians and interdisciplinary team members while ensuring compliance with organizational policies and regulatory requirements. Delivers exceptional customer service and contributes to a respectful, team‑based care environment.
Job Responsibilities- Conducts comprehensive health assessments, including physical examinations, patient histories, and interpretation of diagnostic tests.
- Initiates orders for medications, laboratory studies, diagnostic testing, and treatments within scope of practice and organizational policy.
- Identifies alterations in patient health status and implements appropriate nursing and medical interventions.
- Diagnoses and manages acute and chronic health conditions across organ systems.
- Develops, implements, and evaluates individualized plans of care, considering age‑specific needs.
- Provides patient and family education, counseling, and support regarding treatment plans and health management.
- Coordinates patient care in collaboration with physicians and interdisciplinary team members.
- Facilitates efficient and effective patient flow, including appropriate utilization of resources and discharge planning.
- Participates in interdisciplinary rounds, case conferences, and care planning activities.
- Ensures accurate and timely documentation of patient encounters, procedures, and care plans in accordance with policy.
- Responds to urgent or emergent patient needs and participates in on‑call responsibilities as scheduled.
- Participates in quality improvement activities, including data collection, outcome analysis, and process evaluation.
- Incorporates evidence‑based practice to enhance clinical outcomes and support program development.
- Engages in professional development activities and represents the organization in appropriate community or educational events.
- Performs other duties as assigned.
- Abilities in the use of specialized equipment used in respective area of practice [Required]

- Master's of Nursing [Required]
- 1+ year of related work experience [Required]
- Advanced Practice Registered Nurse (APRN) [Required]
