Psychiatric Nurse Practitioner and Family Nurse Practitioner Port Charlotte, FL | 1099 Contractor | Hybrid Schedule Vita-Cog Community Health is expanding in Port Charlotte, and we’re looking for a Psychiatric Mental Health Nurse Practitioner (PMHNP) and a Family Nurse Practitioner (FNP) to join our group private practice. Our team brings together therapists, neuropsychologists, and other clinicians to provide coordinated care.

We’re seeking practitioners who enjoy collaboration, value thoughtful patient care, and want to help build services in a growing community. The opportunity
- Flexible hybrid schedule with at least 2–3 days of in person availability each week at our Port Charlotte office - 1099 independent contractor position
- Opportunities to collaborate with our therapy and neuropsychology teams
- An established medical director who can serve as the collaborating physician for the PMHNP role We’re looking for candidates who:

- Are board certified in their respective specialty as a PMHNP or FNP
- Hold an active Florida APRN license
- Communicate warmly and clearly with patients and colleagues
- Are comfortable working both independently and as part of a multidisciplinary practice
- Can provide consistent in person availability in Port Charlotte
