Position Overview

The Life Skills Coach (LSC) is a community-based position working directly with individuals who have sustained a brain injury or have some other neurological condition. The LSC position is a role that provides functional supports to clients on an individualized basis designed to assist them in acquiring, retaining and improving the skills necessary to reside independently at home and access a variety of community-based resources.

Basic

Job Duties
  • Provide positive support and act as an advocate for the client.
  • Assist with identifying choices and making informed decisions that involve the client's activities and life.
  • Provide structure and support in household management, such as maintaining cleanliness of the home environment and establishing a safe, healthy and efficient routine.
  • Assist the client with participation in recreation and community activities to promote socialization and establish relationships with peers.
  • Help the client improve community living and integration skills in settings such as shopping, church attendance, sporting events, club participation, classroom support, volunteer/employment support, etc., by providing transportation.
  • Work with clients to learn how to participate in their desired recreational and social activities within their surrounding community.
  • Provide training or assistance with mobility in the home environment, using adaptive aids, and accessing public transportation.
  • Assist with developing effective and appropriate communication and social skills.
  • Operate within organizational guidelines, policies, professional and ethical standards.
  • Maintain organizational confidentiality of information relating to individuals and their families in accordance with HIPAA/company policies.
  • Treat clients, families, co‑workers, and outside parties with respect and dignity, honoring all values and all written and reasonably implied ethical practices of the organization.
  • Use established goals to provide clients with functional supports to acquire, retain and improve independence.
  • Help clients reintegrate and reconnect with their communities to encourage a healthy level of independence.
  • Identify barriers, triggers, or changes in functional status to develop proactive interventions that promote continued independence.
  • Educate clients on how to develop necessary problem‑solving skills and strategies.
  • Accompany clients to locations and appointments in the community to teach them how to assess and utilize community resources and services.
  • Serve as a role model and mentor for clients while maintaining good communication and relationships with the client, their families, and referral sources.
  • Complete daily progress notes and other necessary documentation in compliance with organization policies and provide them to supervisors and the agency's payroll/invoicing email portal.
  • Monitor and appropriately utilize billable hours with clients.
  • Conduct professional behavior at all times.
  • Participate in trainings at the agency and other appropriate training opportunities.
  • Communicate regularly with organization personnel regarding issues and concerns.
  • Participate in team meetings as requested.
  • Perform other duties as assigned to meet program goals or as requested by the agency.
