Maternal Infant Health Public Health Nurse

GENERAL SUMMARY

Under the supervision of the Director/Supervisor of Personal and Community Health Services, the PHN addresses community needs by providing comprehensive nursing skills and services including assessment, diagnosis, teaching, counseling, prevention, and referral services to individuals, families, and groups to promote health throughout the county. Coordinates and participates in the operation of clinics and programs to promote health and wellness to clients in the county.

Maintains patient records and program activity records and prepares required reports. The PHN solves problems with considerable autonomy in working with individuals, families, and groups from the most vulnerable in the community to other professional groups. Prepares and maintains patient records, program activity records, required reports, program policies, and accreditation material. The PHN may also be expected to provide leadership for specific programs.

MATERNAL

INFANT HEALTH NURSE COORDINATOR ESSENTIAL FUNCTIONS
  • Provides in-home comprehensive assessments across multiple domains for pregnant and postpartum women, and mothers of
  • Creates and implements individualized care plans to provide preventative care and wellness education for perinatal women and infants up to one year of
  • Functions as a member of a multidisciplinary team that also includes a licensed social worker, and registered dietician, managed by the MIHP Coordinator.
  • Participates in all quality improvement and quality assurance activities, including reviewing program requirements, data, reports, and chart reviews for achievements and areas for program
  • Manages information by accurately documenting data gathered for billing services and qualifying health risk assessments in accordance with program policies and
  • Coordinates home visiting program services with MDHHS. Communicates between MIHP Consultants and program for any local health department issues pertaining to
  • Directly oversees all program components including client/patient services and grievances. Monitors for various changes and responds accordingly to updates and changes to program structure and requirements related to programing at the MDHHS level. Interprets MDHHS MIHP requirements to apply them to the health department level including writing new and updating existing policies/procedures.
  • Represents the SCHD at meetings, community events and presents to community partners as
  • Coordinates accreditation procedure with MDHHS, prepares material and staff for accreditation process including chart preparation, review of policies, gathering of supporting documentation and pre-review material to be sent to State reviewer.
  • Remains up to date on changes to the MDHHS Medicaid Provider Manual and MDHHS MIHP Operations Guide. Reviews guidelines yearly for changes/updates and implements changes/updates for program compliance and current standards of

EMPLOYMENT QUALIFICATIONS

EDUCATION: Graduation from an accredited School of Nursing. Bachelor’s Degree in Nursing is preferred.

EXPERIENCE: Two years of nursing experience with an emphasis on assessment and communication skills

OTHER REQUIREMENTS : Possession of a current license to practice as a Registered Nurse in the State of Michigan. Possession of a valid Michigan driver’s license.
