Responsible for treatment and management of patients with altered skin integrity due to acute or chronic wounds.
Provides support, education, and guidance to patients and staff while working as part of a team to create a plan of care.
- 1. Develops, revises, and implements appropriate plan of care for patients with skin integrity issues.
- 2. Collaborates with medical staff to create treatment plans for patients with complex wounds.
- 3. Provides education and training to nursing and medical staff related to the assessment and promotion of skin integrity and wound management.
- 4. Evaluates patient and family learning needs and provides education to assure understanding and ability to complete self-care.
- 5. Assures that plan of care is communicated across the care continuum.
- 6. Maintains professional competence through continuing education.
- 7. Performs other related duties as required and directed.

Qualifications Required- Current state license as a Registered Nurse
- Wound Care Experience
- Certified or eligible for certification by the Wound, Ostomy and Continence Nurses (If not currently certified must become certified within twenty-four (24) months of hire/transfer date.)
