We are seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments. You will evaluate patient eligibility, review medical histories, coordinate with the care team, and provide education and emotional support to patients and families. Care is provided in private homes, adult family homes, assisted living, and skilled nursing facilities across Pierce, Thurston, Kitsap, and King counties.

Primary Responsibilities
  • Conduct patient assessments addressing physical, emotional, and spiritual needs
  • Collaborate with physicians and the interdisciplinary team on care plans
  • Educate and support patients and families during hospice admission
  • Complete required documentation accurately and promptly
  • Perform hospice admissions and, when not scheduled, provide routine visits including death visits
  • Stay current with hospice best practices, policies, and regulatory standards
Requirements
  • Active Washington State RN license and BLS certification
  • Valid driver’s license and reliable transportation
  • Minimum 1 year hospice or home health nursing experience preferred
  • Minimum 2–3 years healthcare-related experience required
  • Proficient with EMR systems and healthcare technologies
  • Strong judgment, initiative, and ability to work independently
  • Culturally sensitive and adaptable to diverse patient population
