Position: PROGRAM COORDINATOR - SALISH YOUTH NETWORK COLLABORATIVE (SYNC)

Kitsap County is seeking a compassionate, dedicated, and resourceful professional to fill the vital role of Program Coordinator for the Salish Youth Network Collaborative (SYNC). This position is a vital role in supporting youth and families across Clallam, Jefferson, and Kitsap Counties. As

the SYNC Program Coordinator, you will:
  • Navigate complex behavioral health and youth-serving systems to connect youth and families with appropriate community-based services, supports, and resources across a three-county region.
  • Coordinate individualized care plans by partnering closely with youth and family members to identify strengths, needs, preferences, and short- and long-term goals.
  • Lead Multidisciplinary Team (MDT) meetings involving youth, families, natural supports, providers, and system partners to address complex needs and develop actionable, coordinated support plans.
  • Collaborate with provider agencies, community organizations, and stakeholders to ensure seamless service coordination and continuity of care.
  • Drive family-centered planning efforts that promote engagement, empowerment, and sustainable outcomes for youth and caregivers.
  • Represent the SYNC program and SBH-ASO through regular participation in community outreach and engagement events throughout the Salish region.
  • Act as a liaison between SBH-ASO, youth and families, and community partners to support communication, alignment, and shared accountability.
  • Ensure quality and consistency in behavioral health coordination by supporting system improvement efforts and participating in regional and statewide planning initiatives.
  • Contribute feedback and insights to help identify service gaps, emerging community needs, and opportunities to improve behavioral health systems and operations.
  • Provide administrative and programmatic support to sustain effective program operations and accurate documentation.
What you bring:
  • Educational Excellence: Bachelor's Degree in Social Work or a closely related field of study, from a college or university accredited by an agency recognized by the US Department of Education.
  • Work Experience: At least 1 year of progressively responsible experience in youth behavioral health services. Relevant work experience demonstrating required knowledge, skills, and abilities may substitute for educational requirements. Work experience is essential and cannot be replaced by additional education.
  • Communication/Interpersonal

    Skills:

    • Exceptional communication skills, both verbal and written, with the ability to engage diverse stakeholders and advocate for community needs.
    • Strong interpersonal skills, especially in working with vulnerable populations, are essential to succeed in this role.
  • Additional

    Preferred Qualifications:

    • 2+ years of work experience in behavioral health services, with a focus on youth care
    • One year experience with community engagement and/or group facilitation
    • Experience with youth cross-system navigation, and a strong understanding of behavioral health system planning and improvement.
