DUTIES INCLUDE BUT NOT LIMITED TO:
- Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support
- Research and advocate for appropriate public assistance resources for clients
- Referral to community resources, assistance with socio-economic problems
- Communicate with client’s care team
- Provide crisis intervention (referral to DCF, provider, etc as needed)
- Coordinate treatment planning with specialists and outpatient providers as needed
- Evaluate current patients for appropriate social work services
- Judge relative costs/benefits of potential social service and health care actions and choose the most appropriate one
- Bachelor’s in social work (required)
- Experience working with clients that are affected by issues including neglect, abuse, domestic violence, substance abuse
- Minimum of 2 years prior experience
- State licensure or certification commensurate with degree
