This is a full-time, on-site role for a Social Worker based in Port St. Lucie, FL. The Social Worker is responsible for assessing patients' social, emotional, and support needs to ensure solutions align with their care plans. Responsibilities include collaborating with clinical teams, providing counseling, connecting patients with resources, conducting discharge planning, and advocating for patient rights. The Social Worker will work closely with patients, families, and healthcare providers to deliver compassionate and effective support services.
Qualifications- Proficiency in patient counseling, advocacy, and case management
- Strong communication, interpersonal, and problem-solving skills
- Knowledge of social, emotional, and psychological assessment techniques
- Understanding of healthcare resources, discharge planning, and community services
- Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited institution
- State licensure or certification as required by Florida regulations
- Experience in a healthcare or clinical setting is preferred
- Ability to work effectively as part of a multidisciplinary team
