Transitional Care Coordinator
Coordinates complex discharges and develops a progressive plan to ensure a safe and effective transition to the next care setting. Optimizes the transition from the hospital by actively managing the case and engaging the patient and family to proactively prepare the patient and care team for the complex needs and transition of care. Follows the patient throughout the continuum of care into different facilities and home.
Focuses on medication reconciliation between levels of care and reducing avoidable readmissions. Provides consistent transition overview at each level of care.
- Bachelor’s Degree in Nursing

- 3 years acute care hospital experience
