The Intake Specialist is accountable for providing technical and organizational support in developing, maintaining, and integrating CHS's information system, while sustaining data integrity and reporting. Overall, the Intake Specialist contributes to CHS's strategic objective of reimagining what HOME means by creating spaces where every child and family is supported and has access to the opportunities they need to pursue their dreams.
Primary Job Functions
* Provide technical and organizational support in developing, maintaining, and integrating CHS's information system, while sustaining data integrity and reporting.
* Complete and submit necessary documentation to partners, lead agencies, and/or Case Coordinators.
* Prepare and analyze information for assignment of cases to Case Coordinators or dependency units.
* Maintain and review data in Florida Safe Family Networks (FSFN) and other information systems.
* Set up and maintain cases in the client information system;
Monitor case changes and update the client information system and FSFN accordingly.
* Answer incoming telephone calls from community partners and/or lead agencies for acceptance of new intakes.
* Gather information and documentation to determine whether provision of services is warranted.
* Liaise with community partners, DCF, DOH, and other relevant parties.
* Monitor and report on performance procedures and contract measure targets.
* Receives referral information during agency business hours, which are received by the Department of Children and Families, Law Enforcement and other referral sources in St. Lucie, Martin, Indian River and Okeechobee County covered by the Children's Home Society Child Protection Team.
* Ensure all necessary information is obtained and documented in CPTIS or on the appropriate Intake form as required.
* Provide all intake/referral forms to supervisor daily for assignment.
* Ensure all clients meet program eligibility requirements.
* Staff all referrals with the CPT Medical Provider(s) and Supervisor(s) and schedules forensic, medical exams, interviews and other appointments as needed.
* Notify all involved community partners of appointment dates/times.
* Consult with therapists, psychologist, internal and external medical providers, hospitals, State Attorneys Offices and external community partners regarding case referrals.
* Obtain 8 hours of continuing education per year.
* Report problems with the referral process or individual referral, to Team Coordinator or Program Supervisor, within required timeframe.
* Ensure copies of intake exception reports are sent to the Department of Children and Families daily.
* Participate in community training/outreach as directed by the Team Coordinator.
* Contribute to a positive, engaging work environment.
* Develop a strong knowledge base and stay current on job-related issues, best practices, and trends.
* Participate actively in departmental meetings, training and education, as well as the quality process.
* Comply with CHS's code of conduct, policies, procedures and other obligations.
* Assist with training other team members and providing back-up when necessary.
* Pick up projects on the fly; perform other duties as assigned from time to time.
