This if a full-time position with benefits (based in Port Townsend, Washington).
ESSENTIAL JOB FUNCTIONS Case Worker Responsibilities- Provide coordinated entry and/or direct services to clients experiencing homelessness.
- Deliver holistic, person-centered case management, including housing assessments, life skills development, and crisis intervention.
- Develop individualized service plans with measurable goals and action steps.
- Advocate for clients internally and externally to ensure needs are acknowledged and addressed.
- Travel to meet clients where they are (e.g., schools, public spaces, shelters).
- Facilitate connections to behavioral health, education, employment, and support networks.
- Serve as the first point of contact for individuals and families seeking housing assistance.
- Conduct Coordinated Entry assessments and use HMIS/Olycaps database to enter client data and maintain accurate records.
- Make referrals to internal programs and external housing/service providers.
- Provide diversion, shelter access, and Rapid Rehousing prioritization support.
- Maintain knowledge of Olycap Housing programs, and other agency services.
- Maintain thorough, timely, and accurate case documentation and data entry.
- Attend relevant community meetings, staff trainings, and cross-program collaborations.
- Participate in agency-wide initiatives and support program evaluation efforts.
- Adhere to trauma-informed, culturally responsive, and equity-driven service principles.
- Perform additional duties as assigned.
- Passion for ending homelessness and promoting housing stability.
- Knowledge of homelessness systems, youth development, housing-first models, and Coordinated Entry processes.
- Cultural humility and experience working with individuals from diverse racial, cultural, socio-economic, and experiential backgrounds.
- Strong organizational and communication skills; ability to manage multiple tasks.
- Ability to work collaboratively with a team and independently in the field.
- Crisis management skills and the ability to de-escalate situations calmly and effectively.
- Proficiency with basic computer applications, data systems, and virtual communication platforms.
- Trauma-informed care, motivational interviewing, or harm-reduction training is a plus.
- Preferred:
Relevant lived experience of homelessness or housing instability. - OR:
Bachelor’s degree or relevant work experience in human services, social work, behavioral health, or a related field. - Must pass background checks and possess a valid driver’s license with reliable transportation.
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
Expected hours: 40 per week
