Job SummaryTheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members.
CompensationLPN:
PLUS $2.00per hour
PM and $2.50 per hour
NOC shift differentials

Weekend Warrior Program – choose one of the following 3 levels:
Select Level: base hourly wage +$5.00/hour (every other weekend)
Pro Level: base hourly wage +$7.00/hour (3 weekends per month)
PLUSshift differentials - $2.00 per hour PM shift and $2.50 per hour NOC shift

Key Responsibilities- Provide safe, effective nursing care per care plans and physician orders
- Administer medications and treatments accurately
- Assess residents and report changes promptly
- Document care in the EMR
- Monitors performance and mentor CNAs
- Ensure infection control and safety compliance
- Completes all other duties as assigned.
Required:
- Active
LPN license instate of Wisconsin orlicensedin any U.S state and be eligible for Wisconsin licensure by reciprocity.(in good standing) - CPR certified
- Ability to meet physical and job-related requirements
- Must be English literate andpossessexcellent writtenandverbal communication skills
Preferred:
- Long-term care or skilled nursing experience
- Clinical experience
