Job Description:
## Social Worker New Mexico Christian Children’s Home (NMCCH) seeks a compassionate, dedicated Social Worker to coordinate services and support clients participating in NMCCH programs.
The Social Worker works collaboratively with clients and staff to develop and implement individualized growth plans addressing
** education, physical health, mental health, life skills, and spiritual growth**. Depending on client needs, the Social Worker may meet with clients regularly to provide life coaching, encouragement, accountability, and support as they work toward their goals.### Primary Responsibilities
* Complete client intake and needs assessments.
* Work with clients and staff to develop and monitor individualized growth plans.
* Coordinate services related to education, physical health, mental health, life skills, and spiritual growth.
* Meet regularly with clients to provide life coaching, guidance, and support.
* Connect clients with appropriate life-skills resources and community services.
* Monitor and document client progress during participation in NMCCH programs and provide appropriate follow-up after program completion.
* Assist clients in exploring colleges, universities, trade schools, and other educational or career opportunities.
* Help clients navigate applications, admissions, financial aid, and related educational processes.
* Assist in coordinating appropriate educational accommodations and support services.
* Collaborate with other NMCCH staff to provide consistent, comprehensive support for each client.
