Speech-Language Pathologist (SLP) - Per Diem

Gateway Post Acute

Pay Range: $60-$65 per hour

Position Summary

Gateway Post Acute is seeking a Per Diem Speech-Language Pathologist (SLP) to evaluate and treat residents in our skilled nursing setting. The SLP is responsible for providing comprehensive speech and language services, collaborating with the interdisciplinary team, and maintaining accurate clinical documentation in accordance with state practice acts and payer regulations.

This position plays a vital role in restoring communication, cognition, and swallowing function while supporting residents and families throughout the rehabilitation process.

Essential Duties and Responsibilities
  • Evaluate residents within twenty-four (24) hours or one (1) business day of physician referral. If unable to complete evaluation within that timeframe, document cause for delay in the medical record.
  • Develop individualized treatment plans and obtain physician approval for services.
  • Provide skilled speech therapy services in accordance with physician orders and established care plans.
  • Supervise Speech-Language Pathologist Assistants (SLPAs) in accordance with state practice acts.
  • Communicate regularly with physicians, nursing staff, rehabilitation team members, and families regarding resident progress and care plans.
  • Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as required.
  • Complete all required documentation including evaluations, daily treatment notes, weekly progress notes, and recertifications in compliance with state, federal, and third‑party payer regulations.
  • Record treatment modifications per facility policy and procedure.
  • Provide caregiver and family education to support discharge planning and maintenance programs.
  • Participate in discharge planning and coordination of care.
  • Act as a clinical preceptor for Clinical Fellows (CFY/RFP) and student programs when applicable.
  • Conduct ongoing in‑services and training for facility staff.
  • Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and facility policies.
  • Maintain professional and effective communication with residents, families, and colleagues.
Supervisory Responsibilities
  • Supervise Clinical Fellows (CFY), when applicable.
  • Provide guidance to nursing staff and assist with training Restorative Aides as appropriate.
Qualifications

Education and Experience
  • Master's or Doctoral degree in Speech‑Language Pathology required.
  • Must meet one of the following:
    • Licensed as an SLP in the state of practice;
      or
    • In states without licensure, must have:
      • Completed 350 clock hours of supervised clinical practicum (or currently accumulating supervised clinical experience);
      • Completed at least nine (9) months of supervised full-time SLP services after graduate degree;
      • Successfully passed a national SLP examination approved by the HHS Secretary.
  • Experience in a skilled nursing or post‑acute setting preferred.
  • Proficiency in electronic medical record systems such as Casamba and/or Point Click Care  preferred.
Certificates, Licenses, Registrations
  • Active Speech‑Language Pathologist license in the state of employment.
  • Must maintain license in good standing at all times.
  • Must meet continuing education requirements per state regulations.
Physical Demands

The essential functions of this position require:

  • Frequent standing and walking; occasional sitting.
  • Frequent reaching, pushing, pulling, talking, and hearing.
  • Ability to exert up to 100 pounds of force occasionally, 50 pounds frequently, and 20 pounds constantly to move objects.
  • Occasional climbing, balancing, stooping, kneeling, crouching, or crawling.
  • Close, distance, color, peripheral, and depth perception with the ability to adjust focus.
Work Environment

This position operates within a skilled nursing and rehabilitation environment. The noise level is typically low to moderate. Reasonable accommodations may be made to enable individuals with disabilities to perform essential job functions.
