Licensed Vocational Nurse (w/ Psychiatric/Developmental experience)

Full-time | Various shifts | Overtime as needed
We are seeking a Licensed Vocational Nurse (LVN) with experience in psychiatric and developmental care to provide direct nursing, behavioral training, and support services to individuals with developmental disabilities and severe psychiatric or behavioral conditions in a residential and community-based setting. This role focuses on supporting daily living needs, promoting independence, administering medications, and ensuring a safe, respectful, and therapeutic environment.
LVNs are mandated reporters and play a critical role in supporting consumer safety, dignity, and well-being.
What You’ll Do
Direct Care & Support- Assist consumers in all activities of daily living (bathing, dressing, grooming, housekeeping, dining, toileting, communication, socialization, mobility)
- Train and encourage consumers to maximize independence and life skills (cleaning, budgeting, planning activities)
- Follow individualized nursing and behavioral support plans
- Promote consumer rights, choice-making, and access at all times
- Provide positive intervention and follow individualized activity schedules, including driving and escorting to community outings (dining out, shopping, school, family visits)
- Provide one-to-one supervision/direct observation for consumers on special precautions (elopement, suicide risk, serious medical conditions, self-injurious behavior)
- Respond to emergencies using medical/behavioral intervention techniques, ensuring safety for consumers and staff
- Monitor emergency equipment, medication, and treatment supplies
- Competently use self-defense and hands‑on intervention skills to manage assaultive behaviors
- Document and follow up as needed
- Follow STAR documentation policies for General Event Reports (GER) and clinical record documentation
- Perform nursing procedures within LVN scope (may include ostomy care, feeding techniques, respiratory care, adaptive equipment use, and other authorized procedures)
- Assist physicians or registered nurses as necessary
- Record consumer responses to treatment
- Participate as a member of the Inter-Disciplinary Team (IDT) in developing and implementing Needs and Services Plans
- Assist staff in occupational, recreational, vocational, and educational therapy programs
- Attend review conferences, assist with crisis management goals, long-range goals, skills tasks, behavioral plans, and objectives
- Assist in developing individualized behavioral support and nursing plans, including data collection and behavior interventions
- Promote team concept and maintain open communication between staff, families, and consumers
- Work at various sites as needed to meet staffing requirements
- Work extended hours and/or varying shifts, including mandatory overtime
- Attend and participate in training and clinical supervision for RBT certification
- May serve as Acting Shift Lead in the absence of that person
- May be reassigned to other residences, programs, or projects as needed
- Assist residents with independent or assisted activities of daily living (laundry, bed making, cooking, cleaning, ordering supplies/clothing)
- Care for consumer clothing, personal property, and storage area
- Maintain adaptive equipment through proper handling and cleaning
- Report/correct hazards, unsafe environments, or defective equipment immediately
- Complete routine visual counts and home security checks
- Attend shift and facility meetings
- Residential and community-based setting
- Frequent interaction with individuals who may display challenging behaviors
- Physically active role (must lift up to 50 lbs)
- Hands-on behavior intervention and crisis response required
- Possible travel, overnight stays, rotating shifts, and overtime
- Exposure to communicable diseases, blood‑borne pathogens, medicinal preparations, and other clinical conditions
