Patient Population
The patient population served is all patients, including geriatric, adult, and adolescent. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.
Position Summary
Under the direction of the department Leader(s), the Registered Nurse (R.N.) independently provides holistic, individualized care using the Nursing Process for patients routinely seen in the unit. Demonstrates an advanced level of clinical competency, nursing judgment, teaching skills, and a commitment to improving the quality of patient care. Independently performs general and unit‑specific clinical skills. Contributes to the Sierra View Medical Center Department of Patient Care Services and the nursing profession in a positive manner.
Education,
Training and Experience
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required. Able to administer and dispense all medications as required. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Must be capable of dealing effectively and ethically with co‑workers, physicians, and other hospital personnel. To perform this job successfully, an individual should have basic computer skills.
Licensure and Certifications
Current licensure as a Registered Nurse in the state of California. Must have and maintain current BLS certification or obtain within orientation process, prior to hands‑on patient care. Must have and maintain ACLS certification within six (6) months of assignment to the department. Must have I.V. Conscious Sedation Competency, with six‑month time frame at the discretion of the Department Director.
Responsibilities and Essential Functions
Is responsible for the assessment, treatment and/or care appropriate to the ages of the patients served.Utilizes holistic assessment skills, recognizing subtle, covert or abnormal signs and symptoms that include at least one of the following: involvement of patient/significant other, discharge needs, psychosocial needs, biophysical needs, environmental needs, teaching needs.Identifies nursing diagnosis and interdependent nursing problems and selects applicable nursing care standards to guide patient care. Recognizes issues/concerns that could impact development of mutual goals.Ensures that the Care Plan is specific and unique to the individual patient utilizing Interdependent Nursing Standards and the discharge needs of complex/multi‑system patients.According to established clinical policy, procedure and standards, the R.N. safely performs nursing care including safe administration of medication, troubleshoots equipment and demonstrates skill in clinical problem solving.Ensures that the nursing care provided is consistent with the established Nursing Plan of Care, the Medical Plan of Care and the ages of the patients served.The effectiveness of the Plan of Care is evaluated and communicated through exchange report; recommendations are made for future care and are consistent with current documentation.Communicates evaluation of, or changes in, patient status providing supportive data to the physician or health care team as appropriate.Demonstrates adherence to the Joint Commission "National Patient Safety Goals for Hospitals" as defined by Scope of Practice.Documentation reflects compliance with established Hospital policies and procedures.Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice.Demonstrates an understanding of the use, actions and interactions of drugs used in combination for patients with multi‑system problems.Evaluates teaching effectiveness based on patient response and revises teaching plan to accommodate patient/family comprehension.Completes required unit specific performance criteria checklist.Reviews/Revises/Implements interventions in the plan of care based on changing patient status, progress toward…
