Licensed Vocational Nurse (LVN) – Charge Nurse (Part‑Time Shift)

Sequoia Transitional Care – Porterville, CA

Skilled Nursing Facility, 99 beds

NOC Shift – Part‑Time

Pay range: $30–$35 per hour

• Provide direct nursing care to residents in accordance with individualized care plans
• Serve as the Charge Nurse during the NOC shift, overseeing CNAs and ensuring high‑quality resident care
• Administer medications and treatments as prescribed
• Monitor residents for changes in condition and report to RN Supervisor or physician
• Document all care accurately and timely in the EMR system
• Communicate effectively with residents, families, and interdisciplinary team
• Maintain compliance with safety, infection control, and facility policies
• Respond to emergencies and follow established protocols
• Participate in shift handoffs, training, and care planning

• Current California LVN license in good standing
• CPR certification
• Experience in a SNF or post‑acute setting preferred but not required
• New graduates encouraged – training provided
• Strong written and verbal communication skills
• Computer literate and comfortable with EMR systems
• Compassionate, dependable, and a team player
• Must provide recent medical clearance to work in a SNF
• Must pass background check and be eligible to work in the U.S.
