Registered Nurse DP/SNF Full Time

Shift: Variable 12 hour shifts

Position may qualify for employee referral bonus. 10k sign on bonus with 3+ years experience as RN

PATIENT POPULATION:

The patient population served in the DP/SNF is 21 years old and older. This also includes services which affect facility staff physicians visitors vendors and the general public.

Is responsible for the assessment treatment or care appropriate to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patients needs. Possesses knowledge of growth and development and has an understanding of the range of treatment needed for all patient groups.

Must be able to work normal/scheduled working hours to include Holidays call-backs weeknights weekends and on-call. Agrees to participate as directed in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act (Govt. Code ).

LICENSURE/

CERTIFICATIONS:

An individual must have and maintain current licensure as a Registered Nurse in the state of California. Must have and maintain current BLS certification or obtain within orientation process prior to hands-on patient care.

Responsibilities and

Essential Functions:

Is responsible for the assessment treatment or care appropriate to the ages of the patients served i.e. Adult (from 21 years of age) or Geriatric. Demonstrates the ability to obtain and interpret information in terms of the patients needs possesses knowledge of growth and development and has an understanding of the range of treatment needed for these patient groups. Reviews/revises/records an individualized patient history and assessment and utliizes holistic assessment skills recognizing subtle covert or abnormal signs and symptoms that incorporates each of the following:

a. Involvement of patient/significant other.

b. Discharge needs.

c. Psychosocial needs.

d. Biophysical needs.

e. Environmental needs.

f. Teaching needs.

Demonstrates problem identification skills based on current physical/behavioral science knowledge base.

Identifies nursing diagnosis and interdependent nursing problems and selects applicable nursing care standards to guide patient care.

Care plan reflects:

a. Recognition of issues/concerns that could impact development of mutual goals.

b. Utilizes interdependent nursing standards.

c. The care plan is specific and unique to the individual patient.

d. Discharge needs of complex/multi-system patients.

Routinely reviews/revises the plan of care based on changing patient status progress toward established goals and discharge readiness needs.

According to established clinical policy procedure and standards:

Safely performs nursing care including safe administration of medication.

Demonstrates an understanding of the use actions and interactions of drugs used in combination for patients with multi-system problems.

Troubleshoots equipment.

Demonstrates skill in clinical problem solving.

Implements interventions when the patient is at covert risk of injury or complication.

The effectiveness of the plan of care is evaluated and communicated through report recommendations are made for future care and is consistent with current documentation.

Evaluates effectiveness of nursing care by:

Reviewing/revising the plan of care.

Makes recommendations to health care team members.

Communicates evaluation of or changes in patient status providing supportive data to the physician or health care team as appropriate.
