Join our leadership team as the Care Management Director, where you’ll guide a talented group of professionals and collaborate across clinical teams to make a meaningful impact on patients and families every day. In this dynamic role, you’ll drive change, support real time problem solving, and help shape the future of care delivery at our respected academic medical center – the flagship hospital within our 12 hospital system.

• Education:
  
  Either RN with a BSN and Masters Degree in a Health Care Related field or Masters Degreed professional in Health Care.
• License/
  
  Certifications:
  
  CPHQ preferred. Certification in Case Management preferred.
• Experience:
  
  At least 7 years acute care experience in a complex academic medical center and care management leadership. 5 years of Acute Care Case Management Experience. Minimum 5 years management experience in healthcare or related field required.
