We are seeking an experienced advanced practice provider (APP) to lead our physiatry consultation service in a skilled and subacute rehabilitation setting.
- Provide patient care for the geriatric population, addressing medical barriers such as pain, insomnia, and incontinence.
- Collaborate with interdisciplinary teams including physicians, therapists, and nursing staff to monitor patient progress and facilitate discharge planning.
- Lead daily rounding, documentation, and weekly team meetings to discuss patient status.
- Ideal candidates have at least 2 years of experience in rehabilitation or subacute care, with a background in physiatry, internal medicine, orthopedics, sports medicine, or neurology, and hold active licensure as a Physician Assistant or Nurse Practitioner.
