Assignment Duration:
This is a 13-week contract position.
Schedule:
Day shift 8:00 AM - 5:00 PM. Weekend hours include every other weekend rotation, with options for a permanent schedule.
Job Description:
We are seeking an experienced Registered Nurse for our home health care team. The ideal candidate will have a strong background in patient care management, with the ability to handle a case load of 20-25 patients and execute 5-6 visits per day.
Responsibilities:
- Conduct patient assessments and develop care plans.
- Administer medications and monitor patient health.
- Perform necessary medical procedures, including IV therapy and wound care.
- Utilize electronic medical records (EMR) to document patient information.
- Collaborate with interdisciplinary teams to ensure effective patient care.
Required Experience / Certifications / Licensure:
- Minimum of 2 years of nursing experience, preferably in home health care.
- BLS certification required.
- Experience with Epic and OASIS is preferred.
- Additional skills in IV therapy, tube feeds, and wound care management areas.
