The Home Health Physical Therapy Assistant (PTA) provides skilled physical therapy interventions under the direction and supervision of a licensed Physical Therapist (PT). The PTA delivers care to patients in their homes, following established treatment plans designed to improve mobility, restore function, reduce pain, and promote independence and safety.

Key Responsibilities:

Implement physical therapy treatment plans as directed by the supervising Physical Therapist.

Provide hands‑on therapy techniques and therapeutic exercises to improve patient strength, mobility, endurance, and functional ability.

Educate patients and caregivers on exercises, safety techniques, home programs, and equipment use.

Observe and document patient progress, responses to treatment, and any changes in condition.

Communicate regularly with the supervising PT and interdisciplinary team regarding patient status and care needs.

Assist with assessing the patient’s home environment to support safety and functional independence.

Accurately complete visit notes, documentation, and required reports according to agency standards.

Follow infection control, safety, and patient confidentiality guidelines.

Participate in case conferences, staff meetings, and continuing education as needed.

Qualifications:

Graduate of an accredited Physical Therapist Assistant program.

Current state licensure as a Physical Therapist Assistant.

Minimum of 1 year of clinical experience; home health experience preferred.

Current CPR/BLS certification.

Valid driver’s license, reliable transportation, and auto insurance.

Requirements:

Ability to lift, transfer, and assist patients as needed.

Ability to travel between patient homes.

Occasional bending, standing, kneeling, and prolonged walking.
