The Certified Recovery Mentor provides case management offering a combination of referral linkage, individual counseling, family support, group counseling and case management services to assist clients' successful engagement in treatment and the family's ability to support the client's continued treatment involvement. May work in a residential or outpatient setting. This is a part‑time benefited position working 30 hours per week.

Pay & Benefits$22.18-$28.83 per hour, plus full comprehensive benefits.
Salary Placement NoticeThe posted salary range reflects the compensation potential for this role. Most candidates will be placed between the entry level and midpoint of the range, depending on their experience and qualifications. However, more seasoned professionals with extensive exact or similar experience may be considered for placement toward the higher end of the range. Final salary offers are made in alignment with internal compensation processes in accordance with Oregon Pay Equity laws.
Responsibilities- Screens and selects clients to participate in the mentoring program.
- Develops the service plan for all clients including a reporting plan for each referral.
- Within the first five working days following intake into services, the client and (if appropriate) family members meet with the Recovery Mentor and the Primary Counselor to develop a case management plan.
- Has daily contact with clients for up to the first five weeks after release, helping with engagement in recovery support, housing and employment issues, family adjustment issues, legal, housing, medical and benefit issues. Frequency of contact should gradually decrease to the point of twice‑monthly or monthly contact by the 90th day after release.
- Assists the family in the initial implementation of the plan, maintaining regular communications with the client's primary therapist, collateral providers, community corrections staff, and linking the client system to needed services within the community.
- Determines when to terminate clients from the program because services are no longer necessary, or the client is not responding to the mentor model.
- Documents and maintains client records in the electronic healthcare system within 24 hours of meeting with a client to ensure compliance with agency and contract guidelines and confidentiality.
- Meets regularly with staff from social service agencies to update them on Life Works NW services, identifying unmet services/needs Life Works NW may be able to address; reviews and updates referral and case coordination procedures as needed; obtains referral requests and works with referral sources to remove any barriers to treatment; reports back if clients fail to attend; obtains relevant case information from the referral for the clinician who will be doing the client's intake assessment;
works with referral source and client to facilitate the family's involvement in the assessment and the treatment planning process. - Travels between sites or in the community driving clients when needed.
- High school diploma/GED.
- Must have a current Certified Recovery Mentor Certificate (CRM) with MHACBO.
- Substantial experience with issues involving addiction including the language, values, norms and other attributes of the subculture; the subtleties and linguistic nuances of the differences between honest dialogue and statements made for effect.
- The ability to maintain confidentiality and appropriate boundaries.
- Extensive knowledge of how to efficiently navigate the social service network from the bottom up.
- Must be able to accompany clients throughout their environment.
- Must have knowledge of the legal/corrections system.
- Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
- Requires ability to work some evenings and/or weekends.
- Requires valid driver's license, proof of current automobile insurance coverage, verification of a safe driving record from the DMV, and ability to meet LWNW driving requirements.
