Diabetes Educator – Part‑Time, 24 Hours a Week – Day Schedule
This position supports and travels to Providence Hood River Memorial Hospital and the Portland service area.
Responsibilities- Provide education for clients with diabetes of all ages according to the individualized needs of the client.
- Conduct health and education assessments, develop, implement, and evaluate individualized education plans.
- Take into account psychosocial, economic, and language barriers.
- Perform duties in a classroom or individual outpatient setting in collaboration with a team.
- Accept professional development and community outreach as an integral part of practice.
- Oregon Dietitian License upon hire.
- National CDR Registered Dietitian – Commission on Dietetic Registration upon hire.
- Three (3) years experience in treatment and education of diabetes within the last 2 years.
- Experience with current adult learning theory in relation to diabetes education.
- Experience with the clinical management of diabetes.
- Formal coursework or credentials in principles of adult learning theory and patient‑centered learning.
- Certification in Diabetes Education, or willingness to attain CDCES certification upon hire.
- Experience with diverse ethnic groups.

Pay Range: $40.11 – $62.27 per hour
