Clinical Pharmacist
The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas: (1) Patient Care - The Clinical Pharmacist Specialist is responsible for providing collaborative practice services in collaboration with the healthcare team. (2) Population Management - The Clinical Pharmacist Specialist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.
(3) Preceptor / Educator - The Clinical Pharmacist Specialist will provide education to the healthcare team and patients in written and verbal formats that include formal and informal presentations and publications.

Required Qualifications- Bachelor's Degree - Pharmacy from an accredited School of Pharmacy. OR
- PharmD
- Pharmacy from an accredited School of Pharmacy. - Coursework/Training:
Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experienceor equivalent educ/experience - Oregon Pharmacist License upon hire
- Certification from Board of Pharmacy Specialties within 2 years of hire
- 2 years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
- Board of Pharmacy Preceptor license when eligible and where applicable upon hire
