Job Description

Job Summary

Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co‑leader in an interdisciplinary approach to patient groups.

Job Requirements
  • Doctor of Physical Therapy (DPT) degree (required; license obtained on or after January 2016)
  • Physical Therapy experience:
    Two years preferred
Licenses / Certifications
  • Physical Therapy licensure in the state of practice (required)
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS, HS‑BLS, or RQIBLS) certification (required)
Facility Specific License / Certifications
  • AHA‑BLS or ARC‑BLS (required)
Essential Functions
  • Complete evaluations to meet quality standards and address assessment areas that require attention. Use appropriate clinical techniques and tools and establish individualized treatment plans.
  • Provide therapy interventions using appropriate protocols and techniques, establish individualized care and discharge plans, and ensure plans are communicated to appropriate parties.
  • Complete comprehensive and accurate clinical documentation within established deadlines.
  • Supervise assigned assistants.
  • Perform other job‑related duties as assigned.
