Starting Salary: $71,020-$76,320 DOEClassification:
Exempt, Unrepresented, Full-Time, Benefits EligibleGENERAL RESPONSIBILITIES This position provides leadership, coordination, and supervision of services to assist participants to effectively link to medical, mental health and addiction providers. The Wellness Access Program Manager will supervise the Wellness Access Specialists in various locations in the organization and coordinate with health and addiction providers to promote access to services for our participants.

SPECIFIC RESPONSIBILITIES Internal Program Coordination Oversee operations of the Wellness Access Program, including program design, policies, procedures, staff supervision and support, safety, and quality assurance.

Supervise assigned program staff, including hiring, training, performance appraisals, performance improvement plans, disciplinary actions, separations, and ongoing staff development and support.

Develop and implement programs, capacities, policies, and procedures to meet participant needs and fulfill contract obligations.

Facilitate staff wellness support groups and trauma debriefing as needed.

Coordinate with other program managers to ensure participant needs are met.

External Program Coordination Coordinate with county, city, and community partners to increase capacity and access to services for participants.

Oversee data collection and reporting to stakeholders.

General Responsibilities Maintain accurate, updated, and comprehensive program files that coincide with stakeholder reporting requirements.

Assure accurate and updated documentation of services in Homeless Management Information System (HMIS) Well Sky.

Follow department Best Practices.

Participate in agency-wide initiatives, training, and program planning.

As requested, assist with preparing budgets and overseeing program expenditures.

As requested, participate in all-staff meetings, management meetings, and retreats.

Other duties as assigned.

PERFORMANCE REQUIREMENTSEach Transition Projects employee must:

Acquire First Aid/CPR/AED certification within 30 days of hire and be retested biannually thereafter throughout employment.

Complete Bloodborne Pathogens and Narcan Training within 30 days of hire and be retested annually thereafter throughout employment.

Demonstrate reliability by being present for work as scheduled, arriving and leaving on time and taking breaks in expected time frames.

Interact effectively with persons of diverse ethnic backgrounds, religious views, cultural backgrounds, and sexual orientations and treat each individual with respect and dignity.

Attend agency and program staff meetings.

PHYSICAL/MENTAL REQUIREMENTS The physical and mental demands described here are representative of those that must be met by an employee, with or without accommodation, to successfully perform the essential functions of this job.

Hearing and vision adequate for interaction with participants, interaction with staff and interaction with the general public.

Hearing adequate for telephone work. Vision adequate for close work.

Hand and finger dexterity adequate to operate standard office equipment.

Ability to walk, bend, stand, sit, reach, stoop, pull, squat, kneel, climb stairs and ladders

Sitting at a desk completing paperwork and working on a computer for lengthy periods.

Ability to perform CPR and other emergency life-saving care with the assistance of teammates until emergency responders arrive.

Ability to lift 50 pounds.

WORKING CONDITIONS / SAFETY CONSIDERATIONS Work environments may include exposure to animals/pets across various program sites.

This position may have exposure to blood borne pathogens or other infectious materials during the course of duties. Bloodborne pathogens training, safety protocols, and appropriate protective equipment are provided. The Hepatitis B vaccination is offered at no cost.
